Fall Hazard Management Plan
Objective
The primary objective of Chevron Thailand's Fall Hazard Management
Plan is to prevent incidents and injuries caused by falling through
floor and deck openings and by falling while working at elevations.
Scope
Applies to both Chevron and contract personnel conducting work on
Chevron Thailand facilities including land based locations.
This plan does not apply on drilling rig floors and derricks. Drilling contractors shall employ adequate procedures that are designed to protect their personnel against falls.
For operations on vessels (MODUs, lift boats, and other self-propelled boats), contractors shall employ adequate safety procedures that are designed to guard against falls.
Contractors Fall Protection Plans shall meet or exceed the requirements of Chevron’s Fall Hazard Management Plan.
Key Practices
When Fall Protection/Arrest Equipment Is Required
Personnel must use Fall Protection equipment when performing any of
the following when working:
At elevations of six feet or more, or over the unprotected edge of a work platform, deck, walkway, or floor
In an area where it is possible to fall more than 4 feet through deck and floor openings
On a fixed or portable ladder and the worker is above 6 feet
On the boat landing or Plus 10 level when not surrounded by handrails
On scaffolding that is not Green Tagged. (Note: Individual contractors may require personnel to tie off even if scaffolding is green tagged).
Required Documentation
Prior to beginning work that involves the use of Fall Protection,
personnel must complete or verify the following:
Permit to Work
Fall Protection Checklist
Rescue Plan
Annual inspection forms
A rescue plan must be developed and included in the JSAs for jobs requiring the use of fall protection equipment. Each contractor must be able to provide verification of an annual inspection for all equipment on site. The inspection forms may be hard copy, electronic or must be available upon request from the contractors HES Group.
Training Requirements
Gulf of Thailand contractors fall protection required training
expectations are as follows:
One-Day Fall Protection Class (typically 8 hours) for employees that use fall protection equipment which includes hands on use and demonstration of competency
Two-Day Competent Person Training for at least 1 person per crew on site
Rescue Training for at least 1 person per crew on site
Verification of Contractor Fall Protection Training
On-site documentation may be requested from contractors to provide verification of compliance with Chevron requirements
If discrepancies are identified, a review of the contractor’s fall protection training/training provider will be conducted to ensure that it meets Chevron’s expectations/training criteria.
Fall Protection Training Expectation
Below are guidelines for determining the adequacy of fall hazard
training. In addition to the course content, a written examination
illustrating the students understanding of the materials, and a
hands-on evaluation used to determine competency of each student
should be included.
